For patients who are new to LSCC, the first visit includes an income screening process that determines whether or not you or your family members are eligible for public assistance programs such as Medicaid, CHIP, etc. If you are uninsured and are not eligible for public assistance, the income screening also determines your fee for services based on a sliding fee scale according to Federal Poverty Level guidelines.
For your first appointment, please bring:
- A driver’s license or photo ID
- Your insurance card, if applicable (Medicaid, MAP, CHIP, other)
- IF UNINSURED: a recent pay stub or other proof of income
- IF UNINSURED: proof of address (such as a utility bill)

Before the GCC Dental Clinic opened, I couldn’t find a dentist in the area that would accept Medicaid for my adopted children. We often had to drive long distances to get the care they needed.
