To obtain your medical records from any Lone Star Circle of Care clinic, you must complete and submit a release of information (ROI) form. The completed form should be submitted to your specific clinic location (for example, GCC Pediatric Center). Please allow 3 business days for processing.
To have your medical records released to an LSCC clinic, you must submit a different ROI form to the clinic.
