Within our network of clinics, Lone Star Circle of Care provides the following services:

Integrated Physical and Mental Health Care: It is our experience that a high percentage of visits are by patients without a physical illness or whose actual physical illness is worsened by psychological problems. This makes both diagnosis and treatment difficult. Additionally, patients with chronic physical illnesses are plagued with depression, non-compliance with medical regimens, pain and physical discomfort. The result is poor health and high cost. LSCC delivers an integrated approach in which physical and mental health services are co-located for easy access, coupled with a process for patients to learn about managing their health more effectively through personal choices and lifestyle changes.
Disease Management Programs: Disease management programs are a key part of this fully integrated system. Targeted at better community health and reducing costs to hospitals and our county, our clinics provide disease treatment and management for chronic illnesses such as diabetes and asthma. The goal is to prevent hospitalization, giving our patients the best quality of life they can achieve.
